The organization interest form is used to allow a user which is an organization to express interest in becoming a Heritage Affinity Services charity and/or organization network partner. The user can provide an individual contact name, the name of the organization, the individualís title or position, phone number, email address, fax number, and a method of contact, along with the submission of other comments.
U.S. Merchant Form
The US Merchant form may be used to allow a user to express interest in becoming a Heritage Affinity Services US based merchant network partner. The user can provide an individual contact name, the name of the merchant, the individualís title or position, phone number, email address, type of merchant, fax number, and a method of contact, along with the submission of other comments.
Israeli Merchant form
The Israeli Merchant form may be used to allow a user to express interest in becoming a Heritage Affinity Services Israel based merchant network partner. The user can provide an individual contact name, the name of the merchant, the individualsí title or position, phone number, email address, type of merchant, fax number, and a method of contact, along with the submission of other comments.

Heritage Affinity Services may also collect the following information:
